Medication Pick Up Reminder<\/span><\/b><\/p>

As we near the end of the school year please remember that your child\u2019s medication should be picked up from the clinic by the\u00a0May 30th<\/sup><\/b>. School policy does not allow for medication to remain in the school over the summer. It will be important for you to make arrangements to pick up your child\u2019s medication since medications may not go home with the student. Any medication left in the clinic\u00a0after June 2nd<\/sup>\u00a0at 11:00am<\/b>\u00a0will be discarded according to policy.<\/p>

Please remember to complete medical management plans and medication forms for next year. As you know, medication orders must be completed every school year whether your child remains in the same school or goes to another. If your child has a physical examination during the summer months, this is a good time to get the medication forms completed.<\/p>

Health Screenings<\/strong> Health screenings are held annually for kindergartners, 1st<\/sup>\u00a0graders and 3rd<\/sup>\u00a0graders. As the school year kicks off, be on the lookout for information on screening dates and the screening opt out letter coming home with your student.<\/p>

Does your child need emergency medication at school<\/strong>?<\/strong><\/p>

Does your child need emergency medication at school? Use the forms below based on your child\u2019s medical condition and medication needs. For daily or as needed medications unrelated to a condition requiring a medical management plan, please use the Medication Authorization form, complete with physician’s signature for all medications including over the counter medications.<\/p>

The clinic will be open to receive medication for the 25-26 school year beginning August 7th<\/sup>\u00a0at 9:00am.<\/p>
